Growth Hormone Secretagogues FDA-approved as Egrifta for HIV lipodystrophy

Tesamorelin

Also known as: Egrifta, TH9507

Tesamorelin is a synthetic analog of GHRH (growth hormone-releasing hormone) and the only GHRH analog FDA-approved for clinical use (under the brand name Egrifta, for HIV-associated lipodystrophy). It produces strong, physiologic GH release and is especially noted for reducing visceral (deep belly) fat — making it popular for body recomposition, cognitive benefits, and metabolic health.

Half-Life

~30 min plasma, but sustained GH/IGF-1 elevation

Common Dose

1–2 mg per day

Route

Subcutaneous injection (abdomen)

Frequency

Once daily

Mechanism of Action

Tesamorelin binds to GHRH receptors on the pituitary, stimulating natural pulsatile GH release. The resulting elevation in GH and IGF-1 supports lipolysis (especially visceral fat breakdown), lean muscle preservation, improved skin and tissue quality, and neurocognitive benefits. Unlike CJC-1295 no DAC, it has a longer half-life and produces stronger sustained GH elevation.

Reported Benefits

  • Significant reduction in visceral fat
  • Lean muscle preservation/growth
  • Improved cognition and memory (studied in HIV patients)
  • Better skin elasticity and quality
  • Improved sleep and recovery
  • Lowered triglycerides
  • FDA-approved (in HIV lipodystrophy)

Side Effects

  • !Injection site reactions (most common)
  • !Joint pain or stiffness
  • !Water retention
  • !Numbness or tingling in hands/feet
  • !Mild glucose elevation
  • !Headache
  • !Increased IGF-1 (monitor levels)

Dosage Protocol

Common dose1–2 mg per day
Range1–2 mg per day (clinical dose is 2 mg/day)
FrequencyOnce daily, typically pre-bed
AdministrationSubcutaneous injection (abdomen)

Notes

10 mg vial — reconstitute with 2 mL bac water → 5 mg/mL: 20 units (0.2 mL) = 1 mg. Cycle 8–12 weeks. Consider monitoring IGF-1 and fasting glucose periodically. Best taken on empty stomach.

Contraindications

  • Active malignancy
  • Pregnancy and breastfeeding
  • Diabetic retinopathy
  • Known hypersensitivity
  • Pituitary disease

Storage

Lyophilized: refrigerate. Reconstituted: refrigerate, use within 30 days.
